For the 2018 season, each player and family is required to complete and return the three forms below to participate in LOYL programs.

 

The LOYL Lacrosse registration process requires two steps:

1. Validate your player's US Lacrosse membership ... sign up or renew if necessary
2. Complete the three online forms below prior to taking the field at first practice.

Form 1:

Complete the LOYL Player and Parent Code of Conduct

 

Form 2:

Complete the LOYL Emergency Medical and Program Liability Waiver

 

Form 3:

Complete the Parent/Athlete Concussion Information Sheet
